use for the Monk survey.
I'm sure the file will be somewhat scrambled, so feel free to unscramble
it.
Also, feel free to email me your completed forms, rather than using
snail mail. I prefer to receive all email forms as an attachment in
Word97 format, or some lower WordPerfect format.

Use a different sheet for each site surveyed.
Use the same sheet for multiple nests at one site.
For sites with multiple nests, record all information for each nest.

*A nest is each individual stick structure. A chamber is each cavity in
a nest. For the purpose of this survey, we will presume that each
chamber is active currently.
**Nest site examples include: electrical substation framework, telephone
pole with transformer, telephone pole without transformer, tree, radio
tower, light structure, TV antenna, water tower, building, etc. If the
nest is in a tree, list at least the family (e.g., oak, pine, palm,
Australian-pine, etc.), and if possible, list the tree species (e.g.,
Laurel Oak, Slash Pine, Canary Island Date Palm).
***Optional. Count chambers as time and access allow, but this
information is not necessary. List only the birds observed during the
visit. Extended viewing of a nest to count the number of birds is not
required.
